  • Act 168 Sexual Misconduct/Abuse Disclosure Release – Under Act 126 of 2014, you must complete the header information (name and address of current/former employer) and complete and sign all of Section 1.  You must complete one form for your current employer(s) and one for each of your former employers that were school entities or where  you were employed in a position that had direct contact with children.  There is a copy of this form at the link above and at the bottom of this page.

For All Positions

Employment is contingent upon the satisfactory receipt of all necessary forms and documents.

If you are hired by the district, it will be necessary to have the  School Personnel Health Record Form  including a TB test, completed by your physician prior to beginning employment.   If you already have a recent health record (within one year) you may submit it with your application.
